   Proposal Description:
   Adults with bipolar disorder have been reported to be at fourfold risk of         dementia compared to their age-matched peers, however little is known about       signs of dementia in this population because they are systematically excluded     from dementia research studies. The goals of the proposed research are to         retrospectively describe the longitudinal cognitive and clinical                  characteristics of adults with bipolar disorder who eventually develop            dementia, compared to those who do not. Age- sex- and education-matched adults    with no psychiatric disorders will be used as controls. These data will guide     the development of subsequent prospective studies, and suggest avenues for        exploring interventions in bipolar disorder.                                      The specific objectives are to:                                                   1) Compare the longitudinal trajectories of cognitive change (memory, language,   executive functioning, attention) between patients with and without bipolar       disorder who eventually develop dementia vs. those who do not. Those with         bipolar disorder who develop dementia are expected to show lower initial          cognitive scores and faster declines over time than all other groups.             2) Compare health risk factors (cardiovascular/cerebrovascular disease,           hypertension, hypercholesterolemia, diabetes, alcohol use, smoking, sleep         apnea, medications, TBI) between patients with and without bipolar disorder who   eventually develop dementia vs. those who do not. Those with bipolar disorder     who develop dementia are expected to have more risk factors than all other        groups.                                                                           3) Compare known dementia biomarkers (APOE status Abeta, P-tau, and T-tau CSF     values MRI regional atrophy) between patients with and without bipolar            disorder who eventually develop
